Add New Record (Accept Credit Cards)

Users will enter credit card information in these fields.

1) Card Nick:

2) Name on the Card:

3) Card Type: Users will select the type of credit card from this drop down list.

4) Card Number: Users will write card number in this field.

5) Security Code: Users will enter Security Code of the credit card in this field.

6) Expiration Month: Users will enter expiration month of a credit card in this field.

7) Expiration Year: Users will enter expiration year of a credit card in this field.

8) Is Active: If a credit card is active then tick this check box.

9) Is Default: If it’s the default credit card then users will tick this check box.

10) Create: This button will save your Credit Card record.
